Dear Parent/Guardian,
Thank you for enrolling your child in the Special Olympics UAE Summer Camp. Please read the following waiver carefully and sign below to indicate your understanding and agreement.
1. اللياقة الطبية والمشاركة /Medical Fitness & Participation
أنا الموقع أدناه، بصفتي ولي أمر/الوصي القانوني للمشارك المذكور أعلاه، أؤكد أن طفلي يتمتع بصحة جيدة ولا يعاني من أي حالات طبية تمنعه من المشاركة الآمنة في أنشطة معسكر الصيف للأولمبياد الخاص الإماراتي. وأقر بأن المشاركة في الأنشطة الرياضية والترفيهية تنطوي على مخاطر بطبيعتها، وأتحمل كامل المسؤولية عن التأكد من جاهزية وسلامة طفلي. / I, the undersigned, as the parent/legal guardian of the above-named participant, confirm that my child is physically fit and has no medical condition that would prevent them from safely participating in the activities of the Special Olympics UAE Summer Camp. I acknowledge that participation in sports and recreational activities carries inherent risks, and I accept full responsibility for ensuring my child’s readiness and safety.
2.إخلاء المسؤولية / Waiver of Liability
أقر أنا الموقع أدناه بإعفاء الأولمبياد الخاص الإماراتي، ومسؤوليه، ومديريه، وموظفيه، ومتطوعيه، وشركائه من أي مطالبات أو مسؤوليات أو دعاوى قد تنشأ نتيجة أي إصابة أو حادث أو خسارة قد تحدث خلال المشاركة في أنشطة المعسكر، باستثناء حالات الإهمال الجسيم أو سوء السلوك المتعمد. / I hereby release and discharge Special Olympics UAE, its officers, directors, employees, volunteers, and affiliates from any and all claims, liabilities, or causes of action arising from any injury, accident, or loss that may occur during the camp activities, except in cases of gross negligence or willful misconduct.
